A Case of Thickened Lumbar Ligamentum Flavum



Abstract
A 63 year old man had mild low back pain for nearly 20 years.: The patient developed severe back pain and radiating right thigh pain about a month ago. One week before admission he began to have a difficulty in initiation of urination. On neurological examination straight leg raising produced pain in the thigh at 45¡Æ on the right and 69¡Æ on the left. There was no ,sensory loss, weakness or muscular atrophy. Knee jerk was decreased, on the right and ankle jerk was almost absent on the right. Pantopaque myelography showed partial block and posterior filling defect at L4-5. Total laminectomy of L4-5 was carried out. The dural sac was completely encircled and . compressed posterolaterally by the unusually thickened ligamentum flavum. The thickened ligamentum flavum was totally removed. Postoperatively the patient was..asymptomatic and urination became normal.
